Community Facilitator 

Position Summary

 

The Community Facilitator serves as a community leader for Post-22 participants. The facilitator is crucial in coordinating and facilitating activities and events in both group and individualized settings. A major focus of the role is to lead the development, implementation, and maintenance of Individualized Service Plans (ISP) while ensuring that support is personalized, effective, and fosters growth and independence among participants.

Key Responsibilities 

The Community Facilitator serves as a vital leader focusing on the needs of Post-22

participants and has a wide range of responsibilities, including:

1. Individualized Service Plan (ISP) Management and Development

​

• Lead the development of Individualized Service Plans (ISP) based on the participants' individual needs, strengths, and weaknesses.

• Develop, maintain, and execute the Individualized Support Plans for each attending participant.

• Develop goals, and objectives for each adult that promote ongoing growth and choices.

• Identify and develop data collection for monitoring and reporting ISP progress

• Empower individuals to achieve their goals by identifying strengths, skills, and interests, and creating a personalized plan for employment, social connections, and community contribution.

2. Program and Activity Coordination

​

• Lead vocational on-site training, internships, and employment opportunities.

• Facilitate community engagement through developing and implementing social activities in the community.

• Develop a monthly calendar of events, internships, recreation, and volunteering.

• Work as part of a team to create weekly/monthly schedules of activities to meet diverse participant needs and preferences.

3. Community and Stakeholder Engagement

​

• Build strong cross-functional community relationships.

• Support the intake process by assisting in tours for prospective adults.

• Obtain all necessary paperwork for review and input at team meetings.

​

4. Health, Safety, and Compliance

​

• Identify the individual’s safety skills, supervision needs, and strategies to eliminate risks.

• Understand the medical needs of each adult.

​

5. Communication and Training

​

• Communication with other staff members regarding daily schedules and activities.

• Provide on-the-job training and coaching when applicable.

• Be able to read and understand psychological, medical, behavioral, and performance reports, evaluations, and summaries.

Qualifications

Education and Experience

​

• Bachelor's Degree (University degree) in science, health services, psychology, child development, teaching, or human development.

• 2-4 years of on-hand experience working with intellectually disabled adults.

• Demonstrated experience in leading partnerships within the community.

• Proven track record working in a fast-paced dynamic environment.

• Experience with Code of Massachusetts Regulations (CMR) pertaining to Department of Developmental Services (DDS) governance.

 

Skills & Competencies

​

• Excellent written and verbal communication and can facilitate/articulate recommendations and critical decision points to senior management.

• Ability to develop a strong network across vocational sponsors.

• Ability to provide cross-functional team leadership to maintain alignment and to set and meet collective operational goals.

• Demonstrated ability to develop and maintain a respectful, caring, ethical, and empowering relationship with individuals with intellectual and developmental disabilities

• Sign language and any other adaptive communication experience a plus.

 

Required Licenses and Certifications

​

• Possess a current and valid Class D Motor Vehicle Operator's License.

• Must obtain and maintain Cardiopulmonary Resuscitation (CPR) and First Aid (FA) certification upon successful completion of a training program.

• Experience and/or formal training in Human Rights.

• Crisis Prevention/Intervention Certification (e.g., Safety-Care or CPI) a plus.

Preferred Certifications and Specialized Experience

​

• Experience with Applied Behavior Analysis (ABA) Training or RBT (Registered Behavior Technician) Certification.

• Experience with ISP (Individualized Service Plan) Facilitation Training.

• Experience as a Job Coach/Employment Specialist or with ASET Certification.

• Experience with Assistive Technology (AT) devices for communication, mobility, or task management.

Pre-Employment Screening

​

Recommended candidates must successfully complete pre-employment screening which includes:

• Disabled Persons Protection Commission (DPPC) Abuse Registry Search.

• Criminal background check (CORI) by regulations set forth by the Executive Office of Health and Human Services.

• National Background Checks (fingerprint-based check).​
