Community Facilitator
Position Summary
The Community Facilitator serves as a community leader for Post-22 participants. The facilitator is crucial in coordinating and facilitating activities and events in both group and individualized settings. A major focus of the role is to lead the development, implementation, and maintenance of Individualized Service Plans (ISP) while ensuring that support is personalized, effective, and fosters growth and independence among participants.
